What you will need
Guardians Proof of Residency
This can be a Montana ID, Drivers license or Tribal ID.
Medical Records
Contact UsMedical records documenting the qualifying condition are required for your Medical Cannabis evaluation appointment. You have the option to submit them either via fax or email to our records department.
Qualified Debilitating Conditions
- Cancer, Glaucoma, or positive status for human immunodeficiency virus, or acquired immune deficiency syndrome when the condition or disease results in symptoms that seriously and adversely affect the patient's health status
- Cachexia or wasting syndrome
- Severe chronic pain that is persistent pain of severe intensity that significantly interferes with daily activities as documented by the patient's treating physician
- Intractable nausea or vomiting
- Epilepsy or an intractable seizure disorder
- Multiple sclerosis
- Crohn's disease
- Painful peripheral neuropathy
- A central nervous system disorder resulting in chronic, painful spasticity or muscle spasms
- Admittance into hospice care in accordance with rules adopted by the department
- Post-traumatic stress disorder. (PTSD)
What's Next?
After obtaining the certification from the doctor, a second physician must provide a written certification, confirming that the minor's condition qualifies for medical marijuana use.
Next, the parent or guardian must submit a completed application form as prescribed by the Montana Department of Revenue, along with the minor's written certifications from both physicians. Additionally, proof of legal guardianship must be provided.
