Dr.  appointments  will be ONLY be using Telemedicine New and renewals until specified.  Dr  will continue to serve the patients as are scheduled.

Monday-Friday  8am-3pm

You can see Dr Uphues from your own home, office, phone or computer Monday - Friday 10 am - 5:00 pm or by special arrangement.  No more waiting rooms or traveling in bad weather or taking off work.  You are able to download your medical records in real time to us.  Our site is HIPAA compliant as you would expect from our office.  Your recommendations are downloaded and emailed to you after your appointment.  No downloads to upload to devices. 

New Patients fee $125

New Veterans fee $100

Renewing Dr Uphues Patients $100

Renewing Dr Uphues Veterans $ 75

APPLICATIONS FOR INCREASE OF PRODUCT LIMITATIONS:

With the new implementation of the increase in product limit, there are the following qualifications for patients to be able to be considered.

*You must be an established patient with Dr Uphues.

*You must have a qualifiable condition that warrants the increase, ie; Cancer,  MS, Seizures.  

*You must have an additional visit for consideration as the responsibility for the Doctor has been increased.

*There is a narrative for the patient why they need more as well as the Dr. narrative verifying the need for an increase via medical records on the condition(s).

*Not every patient will be automatically approved for the increase.

*Fee is $100
