- Chronic pain. If this is the condition you are using to get your medical cannabis card,
- you’ll need related MRI’s, X-rays, etc., or have an additional doctor diagnose you with the condition
If you meet the requirements for medical cannabis in Montana, you’ll need to apply on line with the following information downloaded including:
- A Photocopy of your Montana driver’s license in .jpeg format

- 2 x 2 passport type photo in .jpeg format
