The Illinois medical marijuana card requirements are as follows. You must be:

  • At least 18 years of age or older
  • A legal Illinois state resident with proof of residency
  • Diagnosed with a qualifying medical condition as outlined in the list below. According to the Illinois Department of Public Health, you must have one of the qualifying conditions in order to obtain a medical cannabis card in Illinois.

Meeting with a licensed physician is the first step toward getting your medical marijuana card in Illinois. If they see that medical cannabis can help treat your symptoms they will need to fill out the physician certification form and mail it to the Illinois Department of Public Health. 

If you meet the above requirements, you are one step closer to receiving a medical cannabis card. But you will also need to:

  • Fill out the patient application form
  • Pay the application fee
  • Submit a 2x2 inch passport photo
  • Provide proof of your Illinois residency
  • Prove your identity and age (Illinois Driver’s License or US passport)
  • Complete the fingerprint consent form
When you get all these items together, mail everything, including the completed documents and forms, to the Illinois Department of Public Health. Once you are approved, you will need to register with the Illinois Department of Public Health in order to receive your medical marijuana card. Then, you will be able to legally use medical marijuana in Illinois.
